Yes, we accept insurance.

Everyone asks if we accept insurance.  We do, however, we are NOT in network with any provider.  If you have out of network benefits, we will gladly file claims for you.  To verify your benefits, please click the link below and fill out the form.  We will get back to you once we have determined what kind of coverage you have. 

To verify benefits you will need the following information. 

• the name of your insurance company 

• the contact phone number for your insurance company

• the policy holder's date of birth

• the patient's date of birth

• the patient's name

• group number

• id or member number
